Clinical Manager - Transition to Adult Care Program (1.0 FTE - 1 year contract)
The Transition to Adult Care Program is a new initiative that started in 2023 and seeks to support patients with complex, chronic illness, and their caregivers, who often experience challenges with transitioning from pediatric to adult care. As SickKids moves towards a more individualized model of care powered by Precision Child Health, we have a duty to design and implement solutions that bridge Unprecedented Outcomes from childhood to adulthood. The Clinical Manager, Transition to Adult Care Program, plays an instrumental role in the implementation of this novel program by providing leadership to the interdisciplinary team, supporting operations and building/sustaining relationships with internal and external partners.
Reporting to the Executive Director, Brain & Mental Health, the successful candidate will continue to help with the design, implementation and evaluation of an innovative program, with the ultimate goal of delivering an individualized seamless path for youth to transition to adult services.
We are looking for someone who is extremely motivated, flexible, and able to act as a change agent to fulfill this role.
Here's What You'll Get to Do:
- Work as an active member of the program leadership team, collaborating across departments and with other critical internal and external stakeholders in continuing to develop the Transition to Adult Care Program.
- Provide overall operational oversight for the Transition to Adult Care Program, as well as support two ongoing research studies within the program.
- Provide direct management support for team members.
- Work collaboratively with external partners to identify and build strong linkages within the community.
- Implement an ongoing communication strategy, in hospital and community, to increase awareness of the program.
- Chair advisory committees and working groups required to sustain operational activities of the program (collaboratively develop agendas, ensure meetings run efficiently, hold others accountable for action items).
- Liaise with the foundation and support annual reporting to donors.
- Participate in academic work including oral/poster presentations and writing of manuscripts.
- Contribute to the ongoing assessment and evaluation of the early implementation phases of the program and identify opportunities to improve quality and safety.
- Engage in activities to support the sustainability of the program (relationship building, data analysis, building a business case).
- Develop and monitor the program budget in collaboration with the Executive Director and Finance team.
Here's What You'll Need:
- Masters degree in a health-related or business-related field (or evidence of working towards)
- Membership in good standing with a regulated healthcare professional discipline.
- 3-5 years of formal or informal leadership experience in an academic tertiary hospital environment.
- Clinical experience working with paediatric patients with medical complexity and/or rare disease is preferred
- Formal training in project management (PMP) is an asset.
- Experience in program planning, evaluation and QI methodology is an asset.
- A strong business acumen; general understanding of business principles and how they apply to the program's operational needs.
- Strong leadership, interpersonal, organizational, problem solving, critical thinking and conflict resolution skills.
- Demonstrated ability to collaborate and engage constructively with others.
- Ability to advocate, motivate and influence others to gain their support and commitment.
- Commitment to the professional and organizational vision as it pertains to clinical practice and patient care.
